Lumbar Transforaminal Epidural Steroid Injection (Lumbar TFESI)

If you are living with lower back pain that radiates down the leg — making it difficult to stand, walk, or carry out your normal daily activities — a Lumbar Transforaminal Epidural Steroid Injection (Lumbar TFESI) is a highly effective, precisely targeted, non-surgical treatment option that can provide meaningful and lasting relief.

Understanding the Two Types of Lower Back Pain

Accurate diagnosis is the foundation of effective treatment. Lower back pain generally falls into two clinically distinct categories, and the distinction determines which procedure is most appropriate:

  • Localised low back pain — pain confined to the lower back itself, or referring into the hip and buttock without travelling below the knee. This pattern typically originates from inflammation or degeneration of the lumbar facet joints and is generally best addressed with a lumbar medial branch block or facet joint injection rather than TFESI.
  • Radicular lower back pain — in addition to back pain, the patient experiences shooting, burning, or electric-like pain that radiates down the leg, particularly below the knee. This pattern is caused by irritation or compression of a spinal nerve root — and Lumbar TFESI is the procedure of choice for this type of pain, delivering anti-inflammatory medication directly to the affected nerve root.

Correctly identifying which type of pain a patient has is essential before any injection procedure is recommended.

Who Is Lumbar TFESI Suitable For?

This procedure is indicated for patients with leg pain caused by lumbar nerve root compression, arising from:

  • Herniated disc — disc material pressing on a lumbar nerve root, producing pain, numbness, or weakness that radiates into the leg
  • Lumbar spinal stenosis — narrowing of the spinal canal compressing one or more nerve roots, often causing pain or heaviness in both legs with walking
  • Sciatica — radicular pain travelling along the sciatic nerve pathway down the buttock, thigh, and leg, arising from nerve root compression or irritation at the lumbar level

It should For maximum accuracy and safety, patients are required to have a recent Lumbar Spine MRI before the procedure. The MRI allows the physician to precisely identify the level and nature of the nerve compression, and to plan the injection approach accordingly.

What Is a Lumbar TFESI and How Does It Work?

Lumbar TFESI is a minimally invasive, non-surgical procedure in which a fine needle is used to deliver a mixture of local anaesthetic and corticosteroid directly into the neural foramen— the specific opening through which the compressed nerve root exits the spinal canal at each vertebral level.

By placing medication at the exact site of nerve irritation, rather than administering it systemically through oral or intravenous routes, TFESI delivers a high concentration of anti-inflammatory effect directly where it is needed. This reduces nerve root swelling, calms the inflammatory response, and provides rapid, targeted pain relief.

The Procedure (Approximately 15–20 Minutes)

This is a safe, minimally invasive outpatient procedure. General anaesthesia is not required and patients can go home the same day.

  • The patient lies face down on the procedure table; local anaesthetic is applied to the skin to minimise discomfort throughout
  • The physician uses real-time fluoroscopy (live X-ray) combined with contrast dye to navigate the needle to the precise position of the target neural foramen, confirming correct placement before any medication is delivered
  • The local anaesthetic and corticosteroid mixture is injected into the foramen at the affected nerve root level; patients typically feel only mild pressure during this step

The entire procedure takes approximately 15–20 minutes from start to finish.

Expected Outcomes

  • Immediate effect: The local anaesthetic produces rapid pain relief within minutes of the injection, lasting approximately 4–6 hours. This immediate response also serves a diagnostic purpose — confirming that the targeted nerve root is the true source of the patient's leg pain.
  • Sustained effect: The corticosteroid begins reducing nerve root inflammation progressively, reaching its full anti-inflammatory effect within approximately one week.
  • Duration: Patients who respond well typically experience meaningful reduction in leg pain lasting 3 months or longer, with some achieving sustained relief beyond 6 months depending on the severity of the underlying nerve compression and their overall response to treatment.

Aftercare Following Atelocollagen Injection

  • Day of procedure:
    - Do not drive on the day of the procedure — the local anaesthetic may temporarily cause mild leg weakness or reduced sensation; arrange for a companion or use public transport
    - Avoid strenuous physical activity and heavy lifting
  • The following day: Most patients can return to their normal daily activities.
  • Injection site: Mild soreness or aching at the injection site is normal and expected. This typically resolves on its own within 1–2 days.

Supporting Long-Term Recovery
The benefits of TFESI are best maintained and extended when combined with consistent self-care habits:

  • Correct posture — practise proper spinal alignment when sitting and standing; avoid slouching or sustained forward bending.
  • Core strengthening — targeted exercises to build strength in the back extensors and deep abdominal muscles provide better mechanical support for the lumbar spine and reduce the load on the nerve-compressed segments.
  • Regular movement breaks — if your work requires prolonged sitting or standing, take brief breaks every 30–45 minutes to stand, change position, or gently stretch the lower back and legs.
  • Ergonomic support — use a mattress, chair, and pillow that provide appropriate spinal support; proper ergonomics reduce the cumulative mechanical forces on the lumbar spine throughout the day.
