Understanding Peripheral Neuropathic Pain

Have you ever felt a sudden electric shock sensation, a burning sting, or pins and needles in your hands or feet — without any visible wound or injury? These are not sensations you're imagining. They are the hallmark signs of peripheral neuropathic pain: pain caused by damage or dysfunction of the peripheral nerves the network of nerves that connects the spinal cord to the rest of the body.

Unlike ordinary muscle pain, neuropathic pain has a distinct character and complexity. Left untreated, it can significantly impact daily functioning and quality of life.

Causes

Peripheral neuropathic pain can arise from a number of underlying conditions:

  • Diabetes — the most common cause; chronically elevated blood sugar damages the small blood vessels that supply the nerves, leading to gradual nerve deterioration
  • Nerve compression or injury — including a herniated disc pressing on a nerve root, carpal tunnel syndrome (where scar tissue constricts the wrist nerve), or traumatic nerve damage from an accident
  • Viral infections — certain viruses, particularly the varicella-zoster virus (shingles), can leave behind persistent nerve pain even after the rash has fully resolved
  • Nutritional deficiencies — particularly vitamins B1, B6, and B12, which are essential for the formation and maintenance of the myelin sheath that insulates nerve fibres
  • Other contributing factors — including side effects of chemotherapy, chronic alcohol use, and autoimmune conditions where the body's immune system attacks its own nerve tissue

Symptoms

Neuropathic pain has distinctive qualities that set it apart from typical musculoskeletal pain. Common symptoms include:

  • Abnormal pain sensations — sharp, electric shock-like jolts; burning or stinging pain; or a persistent pins-and-needles feeling
  • Allodynia (heightened sensitivity to touch) — even light contact, such as clothing brushing against the skin or a gentle breeze, can trigger significant pain
  • Numbness — a feeling of wearing gloves or socks all the time, or a complete loss of sensation in the affected area
  • Worsening at night — symptoms often intensify after dark, disrupting sleep and leaving patients chronically fatigued
  • Muscle weakness — if the nerves controlling movement are affected, patients may experience difficulty gripping objects, frequent dropping of items, or stumbling when walking

Treatment Options

  • Medication — doctors typically prescribe neuropathic pain agents (such as gabapentin or pregabalin) or certain antidepressants that help modulate pain signalling pathways in the brain and spinal cord
  • Physiotherapy — to rebuild muscle strength, improve function, and stimulate peripheral nerve recovery
  • Managing the underlying condition — for example, strict blood sugar control in diabetic patients is essential to slow further nerve damage

Interventional Procedures
For patients who continue to experience significant pain despite conservative treatment, the following interventional options may be considered:

  • Targeted nerve block injections — local anaesthetic or anti-inflammatory medication delivered precisely to the affected nerve or nerve group
  • Peripheral nerve block — targeted nerve block in areas of concentrated or localised pain
  • Radiofrequency ablation or neuromodulation — uses targeted radiofrequency energy to disable or modulate the nerve responsible for pain signals
  • Peripheral nerve stimulation — a small device delivers low-level electrical impulses to the affected nerve, effectively overriding pain signals before they reach the brain

Prevention

While some causes of peripheral neuropathy are difficult to avoid entirely, the following habits can meaningfully reduce your risk:

  • Eat a nutrient-rich diet — prioritise vegetables, fruit, whole grains, and lean protein to support nerve health and maintain adequate B-vitamin levels
  • Modify daily habits — avoid prolonged crossing of the legs or postures that compress nerves; reduce or eliminate alcohol consumption
  • Exercise regularly — physical activity improves circulation, ensuring nerves receive adequate oxygen and nutrients
  • Minimise nerve-damaging exposures — this includes repetitive strain movements, exposure to neurotoxic chemicals, smoking, and excessive alcohol intake

A Note on Peripheral Neuropathic Pain
Peripheral neuropathic pain is not simply a pain problem — it is a signal that the body's internal communication system is under stress. If you experience recurring sharp pain, burning sensations, or persistent numbness, early consultation with a neurologist alongside a pain management specialist gives you the best chance of an accurate diagnosis and effective treatment. Early intervention consistently leads to better outcomes.
