Lumbar Medial Branch Cooled Radiofrequency Ablation (Cooled RFA for Lower Back Facet Joint Pain)

If chronic lower back pain from degenerative spinal facet joints is affecting your daily life, Lumbar Medial Branch Cooled RFA is an advanced, non-surgical treatment that delivers longer-lasting pain relief than conventional methods — and helps you get back to living fully.

Is Your Back Pain the Right Type for Cooled RFA?

Effective treatment starts with an accurate diagnosis. Back pain generally falls into two main categories, and identifying which applies to you determines the most appropriate treatment path:

  • Localized lower back pain (Facet joint pain) Pain is confined primarily to the lower back, with possible mild radiation into the buttocks — but does not travel below the knee. This pattern typically indicates inflammation or degeneration of the lumbar facet joints. This is the most appropriate type for Cooled RFA.
  • Lower back pain with leg radiation (Radicular pain / Sciatica) Pain radiates from the lower back down the leg, particularly below the knee. This is usually caused by nerve root irritation from a herniated disc or spinal stenosis. For this pattern, Lumbar Epidural Steroid Injection (ESI) is generally the more effective treatment.

What Is Lumbar Medial Branch Cooled RFA?

Cooled RFA is a minimally invasive procedure used to treat chronic lower back pain, particularly pain arising from degenerated lumbar facet joints. Unlike conventional RFA, which uses heat alone to deactivate pain-transmitting nerves, Cooled RFA incorporates an internal water-cooling system within the probe. This allows radiofrequency energy to penetrate more deeply into surrounding tissue — creating a larger "relief zone" compared to standard RFA. The result is more thorough coverage of the medial branch nerves that carry pain signals from the facet joints to the brain.
Cooled RFA technology is approved by both the US FDA and the Thai FDA (อย.), and is used at leading pain management centers worldwide.

Who Is Cooled RFA Suitable For?

  • Patients with chronic lower back pain caused by degenerative lumbar facet joints
  • Patients who have had temporary relief from a lumbar facet joint or medial branch nerve block, and are ready for a longer-lasting solution
  • Patients seeking a non-surgical alternative when other treatments have failed to provide lasting results

The Procedure

The procedure is safe, precise, and typically takes 45–60 minutes depending on the number of nerves being treated.

  • Anesthesia preparation: A local anesthetic is administered along with a mild sedative provided by an anesthesiologist to keep you comfortable throughout the procedure.
  • Real-time image guidance: The physician uses live fluoroscopy (X-ray guidance) to navigate to the exact target nerve locations with high accuracy in real time.
  • Cooled RFA delivery: A small probe is positioned at each nerve site and cooled radiofrequency energy is applied, deactivating the medial branch nerves responsible for transmitting pain signals from the facet joints to the brain.

Expected Outcomes

  • Pain relief: Significant reduction in lower back pain lasting approximately 6 to 24 months, depending on individual response.
  • Reduced medication dependence: Many patients are able to meaningfully reduce or eliminate long-term pain medication use — with real benefits for overall health over time.

Recovery and Aftercare

  • First 1–3 days: Mild soreness at the treatment site is normal and will resolve on its own.
  • Activity: Avoid heavy lifting or strenuous exercise for at least 2–3 days to allow the body to recover fully. Most patients can return to light daily activities the following day.
