Neck Pain Radiating to the Head from the Facet Joints

Neck pain that radiates up toward the head is a common complaint among office workers and older adults. One frequently overlooked cause is dysfunction of the cervical facet joints — small joints that link the vertebrae together along the spine. When these joints degenerate or become inflamed, they can produce neck pain that travels upward and mimics chronic headaches.

What Causes It?

The cervical facet joints are almost constantly in motion, working to control and stabilise the neck throughout the day. Because of this, they are vulnerable to a range of conditions:

•

  Age-related joint degeneration

Injury or trauma — such as a car accident or whiplash

  Degenerative disc disease

  Facet joint osteoarthritis

Poor posture — chronic forward head posture or slouching

Inflammatory spinal conditions such as rheumatoid arthritis or ankylosing spondylitis

  Symptoms

This condition is clinically known as Cervical Facet Joint Syndrome. Symptoms vary based on which joint is affected and the degree of damage, but commonly include:

  • Localised neck pain at the level of the affected joint

  • A dull ache or stiffness that limits how far you can turn, tilt, or flex your neck

  • Headaches, typically at the base of the skull — often referred to as cervicogenic headache

  • Muscle aching or tightness along the neck

  • Tenderness when pressing on the affected joint

  • Referred pain between the shoulder blades or across the upper back

    Treatment Options

    • Initial treatment typically includes:
Medication to reduce pain and inflammation

    • Physiotherapy to restore mobility, correct movement patterns, and strengthen supporting muscles

When conservative care is not sufficient, interventional procedures may be considered:

      • Cervical nerve block — a targeted anaesthetic injection to interrupt pain signals

      • Radiofrequency neurotomy (thermal ablation) — uses heat to deactivate the pain-transmitting nerve

      • Facet joint steroid injection — reduces local inflammation directly at the joint

      • Platelet-Rich Plasma (PRP) injection — promotes tissue healing using the patient's own blood-derived growth factors

The appropriate treatment is selected based on the severity of symptoms, the underlying cause, and how the individual has responded to prior care.

        Prevention

Reducing your risk comes down to consistent daily habits:

        • Maintain good posture while sitting, standing, and working

        • Avoid holding your neck in one position for extended periods — especially prolonged flexion (looking down) or extension (looking up)

        •

Set up your workstation properly: monitor at eye level, chair supporting the natural curve of your spine

Take regular breaks every 30 minutes to stretch your neck and shoulder muscles

Strengthen the deep neck muscles, upper back, and shoulders through targeted exercises, combined with consistent and appropriate stretching

Avoid carrying heavy loads — such as an overloaded backpack — and minimise sudden, jerky, or repetitive neck movements

Choose a pillow that supports the natural curve of your neck — not too high and not too flat
