Diabetic Pain: When Diabetes Goes Beyond Blood Sugar

For people living with diabetes, burning pain, numbness, or electric sensations in the hands and feet are not minor inconveniences to push through. They are warning signs of ongoing nerve damage — and if left inadequately managed, they can progress to affect walking, balance, daily functioning, and significantly increase the risk of serious long-term complications.

How Diabetes Damages the Nerves
Diabetic neuropathy develops as a direct consequence of chronically elevated blood sugar. Two primary mechanisms drive the damage:

1. Microvascular Damage — sustained high blood sugar progressively damages the tiny blood vessels that supply the peripheral nerves. Deprived of adequate oxygen and nutrients, nerve fibres begin to malfunction and lose their ability to self-repair.

2. Nerve Fiber Degeneration — hyperglycaemia triggers oxidative stress and the accumulation of toxic metabolic by-products within nerve cells. This degrades the myelin sheath — the protective insulating layer around nerve fibres — causing signal transmission to become distorted and producing pain, numbness, or burning sensations.

   More Than Just the Feet

A common misconception is that diabetic nerve damage only affects the feet. In reality, it can occur across multiple areas of the body:

   • Hands and arms — numbness, burning, or a sensation of wearing gloves that won't come off

   • Thighs and hips — sudden, severe shooting pain typically on one side; difficulty standing or walking (diabetic amyotrophy)

   • Trunk — pain radiating along the ribs or across the abdomen; frequently misdiagnosed as a problem with another organ system

   • Face and eyes — pain around the eye socket or double vision, caused by involvement of the cranial nerves

Understanding that diabetic neuropathy is not confined to the feet is important for both patients and families — atypical presentations are often missed or treated as unrelated conditions.

     Treatment Options

Standard pain medication and general nerve pain agents can provide partial symptom relief, but they cannot reverse existing nerve damage directly, and carry cumulative side effects with long-term use.

At Bangkok Interventional Clinic, we take a comprehensive, specialist-led approach — using precision pain interventions alongside medical management to restore nerve function and quality of life.

     • Targeted nerve block injection — using ultrasound or fluoroscopy guidance, local anaesthetic and anti-inflammatory medication are delivered precisely to the affected nerve or nerve group, reducing inflammation and modulating abnormal nerve activity

     • Intravenous nutrient infusion therapy — intravenous administration of B-vitamins (particularly B1, B6, and B12) and other neuroprotective agents to support nerve repair and restore the nutritional substrates essential for healthy nerve function

       Prevention

       • Control blood sugar rigorously — maintain HbA1c below 7% through consistent medication adherence, dietary management, and regular glucose monitoring. This is the most effective single measure to prevent diabetic neuropathy from developing or worsening.

       • Exercise regularly — appropriate physical activity improves peripheral circulation, enhances insulin sensitivity, and supports nerve health. Low-impact exercise such as walking, swimming, and cycling is well suited for patients with early neuropathy.

       • Stop smoking — smoking constricts the small blood vessels that supply peripheral nerves, directly accelerating the microvascular damage that drives diabetic neuropathy.

       • Eat a nutritionally complete diet— prioritise foods that support nerve health: vegetables, whole grains, lean protein, and foods rich in B-vitamins. Avoid processed foods with high glycaemic load that contribute to blood sugar volatility.

A Note from Bangkok Interventional Clinic

Diabetic pain is not something you simply have to live with. Our goal is not just to reduce pain — it is to restore nerve function and give you back the confidence to move, work, and live without constant discomfort holding you back.
